Short-Term Host Family Application Form
to be submitted with your short term outbound student's
application by 1 FEBRUARY to
your local Rotary club.
The Rotary International Youth Exchange Host Family application is available on line in Adobe Acrobat PDF format. Click HERE to download. The form is to be filled out by the student with their parents. To get any questions answered regarding short term exchanges, or to have a hard copy of the application form mailed to you, contact Les Crider by email at les.crider@us.army.mil.
